Error with XML, tested out fine in testing tool

I am working to convert an XML message to HL7.   I was able to test using the testing tool without issue.  XML tester, Xlate tester, and route test all worked perfectly.  However, when running the data through the engine, it is causing my process to panic…

Msg sits in state 5 until I blow it away and start the process up… If I leave the message there, it panics the process again.  

qdx version 5.4.1

Any thoughts?
