MSI Region Size Error

I have been setting up a new site.  Things were working fine unitl I added the latest thread which is causing the following errors in the process log:

08/30/2006 15:05:38 [msi :msi :ERR /0:      adt_cmd] msiAllocateTocEntry: Can’t alloc TOC entry for to_ph_cv_a: Not owner

08/30/2006 15:05:38 [msi :msi :ERR /0:      adt_cmd] msiUpdateRegion: failed to add thread to TOC

08/30/2006 15:05:38 [prod:prod:ERR /0:      adt_cmd] Initialization of global shared memory failed

Any idea what I’ve done wrong?

I did some general site clean up things and my error is gone now.  

I deleted the thread that was causing the problem.

Stopped all processes, lock manager, and monitor daemon.

Removed vista.taf file.

Removed monitrShmemFile.

Reinitialized the databases.

Started lock manager, monitor daemon.

Recreated the new thread.

Started the processes, and everything is normal now.
