I will assume so and that you want the outbound MSH-4 to contain some literal like ‘My Facility’.
Just use a COPY Action in the Xlate and for the Source column put =My Facility for the Destination Column just select MSH-4 from the Outbound Message tree.

Or start with a BULKCOPY to copy all the inbound fields to outbound. This will only work if your inbound and outbound variant are equal (or similar), which I guess is the case for you.
If not, you should PATHCOPY all the segments as Shibu pointed out.

I read your replies below saying to add a pathcopy for each segment, im not sure how to even start this but if you did get yours to work any help is appreciated.
I’ve tried everything so far I’m only getting part of the message. I’m very new to this and if someone can help me with a step by step on how to copy the entire mesage. I will appreciate that very much.
