New mktclindex

Have most of you, like me spent hours chasing a tcl proc error only to find out you have more than one of the same proc in the tclIndex?

I rewrote mktclindex to handle this.  Here is a copy.

Put it in $HCIROOT/bin.  Make sure the first line points to proper hcitcl.  Make sure permissions and owner are the same as the old one.

If on Windows, change the name to mktclindex.htc.  If on Unix remove the txt suffix
